Progressing Through Life

Progressing in life could be one of the most difficult things that a person will have to face during their lifetime. The troubles and problems that they encounter helps them to develop as a person, but also puts them through extraordinary pain. Denis Waitley a man known globally for counselling some of the world's most famous athletes, said a quote that could relate back to developing in life. He said â€Å"Don't dwell on what went wrong. Instead, focus on what to do next. Spend your energies on moving forward toward finding the answer.†In the novel The Kite Runner by Khaled Hosseini, developing as a person is portrayed in several different ways through the main character Amir. As a child Amir faced conflict between his father and himself mostly because he was trying to find a way to learn and connect with his father to help him figure out how he wants to live. As he enters his teenage years he experiences evil in this world first hand, something some people will never see. Le arning to control himself and realize how to handle it is a major step for most people.Later when he enters adulthood he begins to take responsibility and puts others ahead of himself. Helping those who cannot help themselves is a considerate and remarkable thing to accomplish in life. During a human’s childhood they face a lot of self realization and this causes most kids to be confused as they are proceeding through the early stages of their lives. Amir's father makes it very challenging for Amir to develop as a child because he does not have very much respect for Amir and does not acknowledge him enough. â€Å"This is grown-ups time.Why don't you go read one of thosebooks of yours? † (Hosseini 3). This quote shows how Amir’s father ignored him like he did not exist. This caused Amir to get very frustrated with his father because all he ever wanted was to be accepted by him. Another point that relates back Baba not loving loving Amir as much as he should, is w hen Baba is talking to Rahim Kahn. They are talking about how Amir never sticks up for himself and Hassan has to do it for him. â€Å"Self defence has nothing to do with meanness. You know what always happens when the neighbourhood boys tease him?Hassan steps in and fends them off. I've seen it with my own eyes. And when they come home, I say to him, † How did Hassan get that scrape on his face? â€Å"†¦ † he fell down† I'm telling you, Rahim, there is something missing in that boy. â€Å"( Hosseini 18). This quote shows us how Baba has very little respect for Amir because he believes that he does not know how to stick up for himself and this causes Baba to ignore him more than he already does. This is not completely Amir’s fault because he has never been taught how to stand up for himself.This is Baba’s job, as a father and role model, to make sure he knows this. There is also a time when Baba tries to reach out to Amir and give him the chanc e to impress Baba but Amir fails to impress him. When Baba puts Amir into soccer, he is hoping that Amir will take an interest in it. Amir likes the idea so he tries it out, but when he does he finds out that he is not good at playing soccer and he cannot get through this athletic challenge. He is unable to keep up and his dad gets frustrated so he takes him out of it and tries to see if watching the soccer will be better for him.For the first few weeks Amir fakes being interested in it and his dad soon catches on that he has no interest in it and wants to give up on him. (Hosseini 15). Amir feels as if he has not inherited any athletic ability from Baba and this makes him feel even more discouraged than he already is. All Amir ever wanted was to get his father’s approval and he fails to do it in almost every situation as a child. As Amir entered his teenage years he faced the evil in the world first hand. Amir witnessed an event that could scar any person for their entire li fe.When Amir saw Hassan get sexually assaulted he did not engage in the situation to try and help his friend Hassan. Amir stood by and watched his best friend surrender to the boys who sexually assaulted him. This situation relates back to the first point in this paragraph because if Baba would have taught Amir how to stick up for himself and his friends, he could have tried to stop or prevent this horrible circumstance. This situation relates to the point because if Amir would have prevented the sexual assault then he would not have felt so guilty and lost his best friend.If this situation never happened it could have change his life completely. After Amir watched his best friend get sexually assaulted, this stalled his growth as a child because he was feeling very guilty and did not know what to do with himself. (Hosseini 61). This is demonstrated during the fight between Amir and Hassan at the pomegranate tree. Amir throws a pomegranate at Hassan trying to get him to throw one ba ck at him because he wants to feel like he is even with Hassan again but Hassan does not react.Instead Hassan smashes one on his forehead to show that he is loyal to Amir and does not want to hurt him like Amir hurt him. (Hosseini 78). These last two examples shows how Amir is developing and maturing as a person because he now knows what he should do in a situation like that if it ever happens again and it also teaches him how he should be loyal to every person you are close to in life. In Amir’s later teenager years his father decides to move to America to get away from all the evil that was starting to happen in Kabul.He thinks that this is a good choice for him and Amir because it will give them the chance to start a new life in a different country. ( Hosseini 93) This is a turning point in Amir's life because he gets a chance to live in a different environment and take all of the knowledge he has gained and apply it to his new life. When Amir enters his adulthood he conti nues to mature and starts to apply everything he has learned through his experiences while growing up as a child and teenager. After Amir finds out that Hassan has passed away, his first instinct is to go and save his son Sohrab.Amir returns back to Kabul and finds out that Sohrab is under the control of the Taliban. As Amir finally finds Sohrab he is faced with the leader of the Taliban which is the man who sexually assaulted Hassan as a child. Amir then sacrifices himself for Sohrab and takes a beating from the leader and escapes with Sohrab later on. (Hosseini 247). This is showing that Amir has figured out that he has to put others before himself sometimes in order to make them happy and do what is right. It is a major step for Amir because he has not done this before.After Amir rescues Sorhab from the Taliban he brings him to a hotel so they can stay together until he brings him back to America. The morning after they settle at the hotel, Sohrab runs away to a Mosque and Amir h ad no idea where he went. Amir then started to panic because he knew that Sohrab was now his responsibility. Amir then asks the man at the front desk several questions until he mentions the Mosque and Amir immediately ran there hoping to find Sohrab. He finds Sohrab there and has a huge weight lifted off of his shoulders because he has found him, and knows that he is okay. (Hosseini 271).As Amir stays with Sohrab he is trying to get the papers which allows him to enter America. When Amir first meets the man trying to help him get Sohrab into America he faces a major issue that becomes an obstacle. The man tells Amir that Sohrab will not be able to leave Kabul until Amir gets proof that he is the uncle of Sohrab. (Hosseini 290). This brave challenge would have been extremely difficult for Amir to handle previously but since he is now very responsible and mentally stronger, he now works hard and does what is necessary to make it possible for Sohrab to come back to America with Amir.Am ir has grown and developed significantly since his childhood and now realizes how he can have in impact on other people’s lives if he really puts his mind to it. Mary Engelbreit once said, â€Å"If you don’t like something change it; if you can’t change it, change the way you think about it. † Developing in life is a very difficult challenge for most people, but for some it is something that they can do overnight. In this novel it is close to impossible for Amir to develop in the main stages of his life because of the type of person he is.In his childhood he tries to get his fathers approval, but later finds out that it will be a lot more challenging than it may seem. As he enters his teenage years Amir exposed to the worlds evil first hand. He witness' an event that he could have changed but fails to do so because of his lack of knowledge. Then when Amir becomes an adult he finally starts to figure out how to take the responsibility for others and put ot hers in front of himself in order to make them happy. Amir finally develops in life and puts everything that he has learned to the test. Amir has come a long way and now can say that he is a fully developed man.

Death of a Salesman Essay

How effective is the ending? Who do we sympathise with? ‘Death of a Salesman’ is a play based on the examination of American life and the effects of the American Dream. Arthur Miller wrote it in 1948, and shows how life was when living a life based on the American Dream. Willy, a failed salesman spent his life following the American Dream, effecting all of those around him and leading him to suicide. The ending of the play greatly effects the feeling and sympathy given for all of the characters. It is a very effective way of ending the play, as the last  scenes seem to link very well to show Willy’s funeral. It allows the characters true feelings to be shown, and the full extent of the American dream is revealed. The ending of the play shows the true feelings and actions of the characters, in particular Happy. Happy shows his true feelings towards his father and his life; throughout the play this has not been very well shown and Happy has been left a mysterious character. That is until the Requiem, at this point Happy shows his belief in his father and how much he truly looked up to him, â€Å"He had no right to do that. There was no necessity for it. We would’ve helped him. † Of the two brothers Happy still believes in the American dream, and has fallen for its poor existence. He still supports it after seeing what has happened to his father and the life he has lived. This is shown at the very end of the play, † I’m not licked that easily. I’m staying right in this city, and I’m gonna beat this racket! † I think Happy believes that he owes it to his father to follow in his footsteps, so that he has at least one successful son. Happy spent his life trying to prove himself to his father and mother, as no attention was paid to him,†Happy: I’m gonna get married, Mom. I wanted to tell you. Linda: Go to sleep, dear. Happy (going): I just wanted to tell you. † At the beginning of the play Happy tends to fight for the attention of his parents to over come Biff’s success. He is always trying to prove himself and obviously cares for his father. Towards the beginning of the play when Linda and Biff begin to worry about Willy’s mental state Happy always seems to be the one to stand up for him and care, â€Å"He’s going to get his licence taken away if he keeps that up. I’m getting nervous about him, y’know, Biff? â€Å". As the play progress Happy grows as a person. Although it is not very visible Happy does try to keep the family together. As he sees himself as successful in Willy’s eyes he tries to help Biff become successful by setting up a business together, â€Å"Wait! We form two basketball teams, see? Two water-polo teams. We play each other. It’s million dollars’ worth of publicity. Two brothers, see? ‘The Loman Brothers’. Baby, we could sell sporting goods! † This all shows that Willy was wrong about his sons, Happy is the son with the courage and strength to be part of the American dream. He is the son that will follow on from Willy and either be a success or a failure. Within the ending of the play Biff’s true character is also shown. During the play he has a moment of self-realisation, showing him who he really is and how he is living a life of lies. The ending develops this and shows him as who he really is. Just before Willy’s death Biff confronts Willy about everything in his life. Biff tries to explain to Willy about how he is living a lie leading to an argument and a loving moment between the two. This shows how the American Dream truly effect those around Willy and brings the play to a good ending. It shows that although the two men had a moment of forgiveness and happiness together it was still not good enough for Willy. As Biff still had no success and did not wish for it Willy still felt a failure. Willy expected Biff to succeed in many ways, from the beginning of the play this is visible. As a child Biff was popular in school and was succeeding in sport although his school grades were dropping. His popularity is visible within the beginning of the play as Willy looks back on his life, â€Å"Biff [goes through the wall-line of kitchen to doorway at the back and calls down]: Fellas! Everybody sweep out the furnace room! I’ll be right down! Voices: All right! Okay, Biff. Biff: George and Sam and Frank, come out back! We’re hangin’ up the wash! Come on, Hap, on the double! † Willy seemed to care more about Biff’s popularity and friends rather than his success in school. This is the influence of the American Dream upon Willy’s life and family. As Biff grows older this still seems to be the case, although Willy also seems to expect Biff to be earning a lot of money. This is Willy’s idea of success; it is more materialistic rather than personal goals. During the end of the play Biff makes a number of short speeches full of his thoughts and feelings. I think this is very effective for the end of the play as it shows emotion in a different way. An example of one of the speeches is, â€Å"Pop, get this now, will you? Every time I’ve left it’s been a fight that sent me out of here. Today I realised some thing about myself and I tried to explain it to you and I – I think I’m just not smart enough to make any sense out of it for you. To hell with those whose fault it is or anything like that. † This shows how Biff feels about himself and his father; it is the beginning of the argument and shows great feeling. The very end of the play is shown as Willy’s funeral, it is very small and consists of only 5 or 6 people. This is very much different to what Willy expected, he hoped for a large funeral with every one he had ever known or met. These expectations were all due to the falsity of the American Dream and the funeral shows this in many ways. It is a very effective ending as  the funeral puts life in prospective for the others and shows their true character. It shows that Biff really cared for his father, as he paid little notice to the amount of people there, he did not need to be impressed with popularity. Where as Happy was concerned about the funeral and still believed in his fathers dream, â€Å"I’m gonna show you and everybody else that Willy Loman did not die in vain. He had a good dream. It’s the only dream you can have – to come out number – one-man. † There are three other main characters within the play that show the contrasts and other parts of the harsh reality of the American dream. Howard is Willy’s boss; he cares for money and not people and is also very fascinated with technology. He shows the growing business world and the effects of the American Dream as a truly successful man. His fascination with technology tends to over power his care for people. For example Howard has purchased a Dictaphone and is showing it to Willy whilst he is trying to talk, â€Å"Willy: It certainly is a – Howard: Sh, for God’s sake! His Son: ‘It’s nine o’clock, Bulova watch time. So I have to go to sleep. ‘

Appreciation of Shakespeare’s sonnet 18

His early plays were mainly comedies and istories, genres he raised to the peak of sophistication and artistry by the end of the sixteenth century. He then wrote mainly tragedies until about 1608, including Hamlet, King Lear, and Macbeth, considered some of the finest works in the English language. In his last phase, he wrote tragicomedies, also known as romances, and collaborated with other playwrights . Shakespeare's reputation did not rise to its present heights until the nineteenth century.The Romantics, in particular, acclaimed Shakespeare's genius, and the Victorians worshipped Shakespeare with a reverence that George Bernard Shaw called â€Å"bardolatry†. In the twentieth century, his work was repeatedly adopted and rediscovered by new movements in scholarship and performance. His plays remain highly popular today and are constantly studied, performed and reinterpreted in diverse cultural and political contexts throughout the world.His plays have been translated into ev ery major living language and are performed more often than those of any other playwright. Of Shakespeare's sonnets in the text, Sonnet 18 is one of the most moving lyric poems that I have ever read. There is great use of imagery within the sonnet. This is not to say that the rest of the poems in the ook were not good, but this to me was the best, most interesting, and most beautiful of them. Theories of Nursing - Research Paper Example One of the central tenets of Benner’s work is the idea that care forms the central aspect of nursing, that it allows a nurse to better connect to a patient, to better administer care, and to better see possible problems and situations before they arise (Alligood, 2002, 157). Though I agree to a certain extent with the primacy of care to any practical nursing environment, I believe that care can only be of actual use to a nurse if he or she has the advantage of experience to guide them in how to best support their patients. Much of my recent practice has been as a primary triage nurse for an (unfortunately) over-crowded emergency room. This emergency department has fifty-three adult patient beds and it is up to me to decide how they are best used in most situations, only changing significantly when patients develop new symptoms or problems become apparent after the triage process has already begun. In a recent experience I had fifty-two beds filled with extremely high priority patients, something of a nightmare, and nearly simultaneously had six patients presenting the extremely distressing symptom of chest pains, all describing them in nearly identical ways. I obviously had only one bed to give until other beds cleared from patients being released or transferred, and had to make one of the most difficult decisions I ever had to make as a triage nurse. I realize in retrospect that I naturally applied many of Benner’s theories of Person, Environment, Health and Nursing to the situation at hand without necessarily realizing it at the time. Obviously, in a triage situation, two of the aspects of this theory most directly applied to what I was doing in practice – Health and Nursing. In a triage setting two things are important: identifying the patient most in need of care (or in extreme triage situations, the person to whom care will be most useful) and as a nurse finding the best and most efficient way to identify the best and most effective me thods of allowing them to receive that care. So I had to identify which of these people was the most unwell, and how I could best act to make them better, whilst simultaneously trying to get the other five the care they needed as fast as humanly possible. My education certainly played a significant role in my decision. In nursing school, I learned how to differentiate between a wide variety of chest pains, to use specialized language (such as throbbing, stabbing, dull, sharp and so on) to help weed out the possible causes of a wide variety of pains, and thus by knowing their causes apply the best care to the group of six as possible, which in this case meant getting the most serious case a bed as soon as possible. Likewise, I had an academic knowledge of how to use pain scales to help identify patients based on their level of pain from one to ten, with one being nearly nothing and ten being the worst level of pain possible. Using this academic knowledge I was able to quickly dismiss two of the cases as most likely being caused by acid reflux (burning pain high in the esophagus with relatively moderate pain scale) and patients who were, though taking a good precaution by seeking medical care, probably not in need of emergency services in an urgent fashion. Comparing different theoretical approaches for two disabilities - Research Paper Example Definition Dyspraxia or developmental dyspraxia is 'an impairment or immaturity of the organisation of movement' (Dyspraxia Foundation, 2009). It is also known as 'developmental coordination disorder' and 'clumsy child syndrome'. According to the American Psychiatric Association (2001), developmental dyspraxia is "marked impairment of motor coordination which significantly interferes with academic achievement or activities of daily living and is not due to a general medical condition." Dyslexia, also known as developmental reading disorder is the most common childhood learning disability which primarily manifests in school going children. There is no single definition that exists for dyslexia. However, those who know about the condition and have worked with dyslexic children are in a position to easily identify the condition. According to the NINDS (2009), dyslexia is defined as "a brain-based type of learning disability that specifically impairs a person's ability to read." The Worl d Federation of Neurology (cited in British Dyslexics, 2009) defines this condition as "a disorder manifested by difficulties in learning to read, despite conventional instruction, adequate intelligence and socio-cultural opportunity." The British Dyslexics (2009) puts the definition as "Intelligent, bright or even gifted individuals, that for no obvious reason, struggle to learn through the medium of written or spoken language." Other commonly used definitions include "learning difficulty characterized by problems with written or spoken language such as reading, writing, spelling, speaking, or listening" and "congenital disturbance of brain function causing a variety of learning difficulties, especially relating to reading, writing and spelling" (British Dyslexics, 2009). Magnitude of problem Developmental dyspraxia is mostly diagnosed in childhood. It occurs in 2 to 5% of children between the ages 5 to 11. It affects boys more than girls (Dorset Country Council, 1998). Infact, boy s account for 70% of dyspraxic cases. The most important symptoms of dyspraxia are poor motor coordination, clumsiness, lack of movement fluency and the difficulty in application of motor skills mastered in one setting to another setting. Dyslexia affects about 80% of all individuals diagnosed to be having learning disabilities (Shaywitz et al, 2007). When only dyslexia is present, the condition is characterized by an unexpected difficulty in reading in otherwise intelligent children who are motivated and have ample learning opportunities considered necessary for accurate and fluent reading (Fisher et al, 2001). The incidence of dyslexia has been estimated to be about 8 to 15% (Earl, 2006). Although epidemiological studies show equal incidence of this condition in both girls and boys, boys are 3-4 times more likely to be referred for reading problems (Tynan, 2006). All dyslexic children share some common aspects enabling them to be easily diagnosed. The increased awareness of dyslex ia has led to many parent and teacher teaching programs, mostly based on multi-sensory learning. Mechanism of disability The exact cause of developmental dyspraxia is unknown. Neurological immaturity is the most probable cause of this condition. Failure of the neurones of the right hemisphere of the brain to form precise pathways during the development of brain leads to lack of accuracy in the instruction of the brain for performing movements (Bowens and Smith, 1999).

Hard systems as Checkland & Holwell (1997) explain included the natural systems. Natural systems are th ose that are not manmade, which include the biological systems, weather, volcanoes and other natural phenomena. Logistics and ticketing are susceptible to interruptions as a result of the hard systems such as weather and disruptions of the equipment. This was taken care of in risk awareness. On the other hand, soft systems refer to those systems that involved the team members’ behaviors and beliefs. Human beings are unpredictable in most cases in the physical aspect and the way they think. Soft systems refer to the beliefs, attitudes, behavior values, opinions that describe each of the team members. Chronic Urinary Tract Infections Treatment Case Nursing Essay RP, a 72-year-old Caucasian female, was brought to the emergency room (E.R) from Hillcrest Adult Foster Care via ambulance. Preadmission report to F-300 stated that she came to E.R confused, lethargic and weak, complaining of acute pain upon urination. She also urinated small amounts ( Upon arrival to F-300 RPs labs revealed significantly lowered thyroid stimulation hormone (TSH), a lowered red blood cell (RBC) count, along with decreased hemoglobin and hematocrit (HH). Her white blood (WBC) count was elevated, as expected with a urinary tract infection (UTI). Primary Diagnosis and Priority Secondary diagnosis The primary medical diagnosis was chronic urinary tract infections, with a secondary diagnosis of hypothyroidism. Patient History RP has a history of chronic UTIs, hypothyroidism, chronic kidney disease (stage III), and chronic anemia. She has several admissions the past few months related mainly to UTIs and dehydration. PATHOPHYSIOLOGY/ETILOGY OF THE PRIMARY DIAGNOSIS AND PRIORITY SECONDARY DIAGNOSIS UTIs are the results of infection, mainly from bacteria, fungi, viruses, or parasites. This leads to a condition referred to as cystitis (inflammation of the bladder). Most common of the UTIs are from infection known as infectious cystitis. A bacterium, the most common cause of infectious cystitis, is from the external urethra, this spreads inwards to the bladder (distal to proximal). Once a bacterium migrates to the bladder and starts the process of growing, it can migrate to other parts of the body (Ignatavicius Workman, 2006). UTIs present with symptoms that include frequency, urgency, retention, burning, foul odor, and incontinence. According to Wikipedia, four stages must be met in order for bacteria to grow. These include an infectious agent, one that must be able to grow, multiply and enter the body. Once we have the agent, the bacteria must find a portal of entry, (how it enters the body). Once in the body it needs an adequate reservoir, a place where organisms can thrive and multiply and the susceptibility of host, how bacteria affects the body after entry. Determents include age, overall health and other co-morbidities of the host and the susceptibility to pathogens. Most UTIs first grow in the perineal area due to irritation; in RPs case this may have been caused by lack of personal hygiene due to being overly fatigued, brought on by her hypothyroidism. Minton (2009) stated, Thyroid is the most important hormone in the body. Because it stimulates the production of cellular energy, production of all other hormones will be negatively impacted when thyroid hormone levels are less than optimal. Every aspect of health is affected by low thyroid function. Hypothyroidism is signaled by fatigue and loss of energy. According to textbook, hypothyroidism is the result of decreased metabolism from low levels of thyroid hormones (THs). Low levels of THs may be a result of several different reasons: thyroid cells may fail to produce sufficient amounts, or the cells themselves are damaged. The patient may not be ingesting enough of the substances needed to make THs, especially iodide and tyrosine. Low levels of THs affect most tissue and organs causing decreased cellular energy. The patient could become confused, lethargic, and have slowing of intellectual functions. ACTUAL OR POTENTIAL IMPACT OF RELEVANT MEDICAL HISTORY ON THE PRIMARY DIAGNOSIS AND PRIORITY SECONDARY DIAGNOSIS RP has a history of chronic UTIs that has caused numerous admits to the hospital over the last few months. UTIs that are left untreated may and can cause damage to the kidneys and start the process of renal failure. Chronic infection of the kidneys causes scar tissue, which decreases the function of the renal system. With renal failure, the body cannot rid itself of certain toxins that may contribute to UTIs. Chronic UTIs may lead to chronic pyelonephritis, repeated upper tract infection of bacteria that migrates from the bladder superior (toward the kidney) to the kidneys. RPs diagnosis of chronic renal failure may be heightened by the chronic UTIs. Hypothyroidism and anemia, that cause fatigue and loss of energy, will discourage you from providing adequate hygiene. This encourages bacterial growth, which may start the chain of events leading up to a UTI. RP resides at an extended care facility that may not have adequate staff to assure that she receives the necessary amounts of iodide and tyrosine needed stimulate TH production. She is also very confused which would decrease her understanding of the importance of proper nutritional intake. MEDICAL MANAGEMENT: CHRONIC URINARY TRACT INFECTIONS And Hypothyroidism A urine sample must be obtained either by a clean catch method or if client unable, by straight catheter method. A catheter method was used on RP, due to her confusion and retention. Urinalysis testing for leukocyte esterase (n=negative) and nitrate (n=none), along with a WBC (n= 0-4) are specifically for diagnosis of a UTIs (Mosbys 2006). Normal urine should appear clear with a yellow tint present (Mosbys 2006). A noticeable smell should not be present in uninfected urine; RPs urine had a pungent odor noted. RPs leukocyte esterase was 3+ and her nitrate was positive. WBCs were to numerous to count. These results determined that RP did have a UTI. Blood work was noted that RPs thyroid stimulating hormone (TSH) was 0.05 (n=2-10). This confirmed the diagnosis of hypothyroidism. NURSING MANAGEMENT: URINARY TRACT INFECTION Recommended textbook intervention is to monitor for signs of UTIs. This includes, but not limited to: frequency, urgency, dysuria, incontinence, pyuria. In some older adults the only sign may be an increase in mental confusion or frequent, unexplained falls. RP was on Q-4 hour vital sign assessment, paying close attention to any increase in temperature and/or heart rate. The patient remained afebrile throughout admission. Heart rate remained between 80-90 beats per minute. Respiratory rate remained between 16-20 breaths per minute. Due to her confusion, bed alarms and padded side rails were used. She had a high fall risk assessment; therefore fall risk precautions were implemented upon admission. She was placed on strict intake and output (IO), and her fluids were monitored. She was instructed to drink required amount of fluid per day (1500ml). Staff member offered bedside commode every two hours to encouraging voiding. She and family members were informed as to the importance of RP to remain dry. This will limit the environmental factors needed for bacterial growth. RN (preceptor) communicated with adult foster care on the needs of the client after discharge. Lab results were also monitored closely for any change that would indicate worsening infection. PHARMACOLOGICAL MANAGEMENT: URINARY TRACT INFECTION AND HYPOTHYROIDISM Textbook recommends medications that treat bacteria and the promotion of client comfort. Cure is dependent on the antibiotic level achieved in the urine. Long-term antibiotic therapy is recommended for chronic UTIs (Ignatavicius Workman, 2006). RP received, via intravenous therapy (IV), Azactam (antibiotic) per Dr. order, to treat her infection. Paroxetine mesylate (antianxiety agent, antidepressant) 30 mg by mouth daily, used to decrease anxiety. Also given Levothyroxine (hormone) as a thyroid supplement. PROVIDER AND MANAGER ROLE: NURSING CARE PLAN Priority Nursing Diagnosis P Impaired urinary elimination R Incontinence due to urinary tract infection C Acute lower tract pain 7/10 upon urination, Priority Patient Goal The patient will be able to state absence of pain or excessive urination by discharge AEB: Pain upon urination 0/10, Decreased urge to void, Empting bladder completely Three Priority Nursing Interventions The nurse will monitor patient for incomplete emptying of bladder by using bladder scan post void. The nurse will encourage patient to drink water at each meal. The nurse will instruct patient on the signs and symptoms of a urinary tract infection. Evaluation of Progress toward Patient Goal The patient was able to recite some signs of UTIs to nurse. She recognized pain on urination and foul odor as signs of an infection. The patient was reluctant to drink water at any one time. She continued to have incontinent periods, but this was less frequent on day of discharge. The patient partially met the goals put forth for her. PROVIDER AND MANAGER ROLE Member of the discipline and the Role of the Multi-Disciplinary Team As a member of the discipline I was inclined to have verbal interactions with the ER staff before RP had arrived to F-300. Consulted with housekeeping to assure the RPs room was cleaned and ready for admission. Ward clerk assured that tests were ordered. Dietician was consulted to help with nutritional needs. Discharged planner made transportation arrangement to return patient to long care facility. Social worker discussed local community resources of support with the family. Provider of Care Role As provider of care for this patient, I kept this patient cleaned and dry when incontinent. I utilized the fall risk precautions to provided safety. Implemented steps to encourage patient to accept and understand a bladder training routine. Monitored patient labs and reported abnormal results to the doctor. Manager of Care Role As manager of care I evaluated the client for her fall risk and implemented fall risk precautions. Delegated the placement of bed alarms and tabs to the LPN assigned to us. My preceptor and I discussed the possible ways to teach client prevention of developing UTIs. Collaborated with nursing home staff on ways to improve clients care at extended care facility. Growth in the Manager of Care Role In completing these tasks I was able to see the complete picture of the patient. I was educated on the disease process and the effect other co-morbidities had on providing an environment that encourages an infection. In caring for RP I was able to developed and strengthen my delegating skills, learning to rely on others and not solely on myself.

With the previous and recent events of natural disasters that have continually to take place across the world, there have been some challenges that the tourist generating and destination areas have faced. These natural disasters have brought about devastating impacts to these areas which entirely or partly depend on tourism as a source of generating income to survive. It is important to note that natural disasters continue to bring about destructive impacts to the supply and demand of both generating and destination areas, as well as affecting the normal lives of people who are forced to adjust to these changes after a disaster. This essay will consider previous natural disasters that have occurred internationally and will examine t he impacts it has on the supply and demand side of tourism with given examples. There are three categories in this essay. Firstly, it will focus on previous natural disasters that have taken place internationally, and then individually for each natural disaster, it will consider the impacts on the supply side; such as accommodation, transportation and attractions, and then it will examine the impacts on the demand side; such as personal income, destination image and health risks. Finally, this essay will end with a conclusion that natural disasters have brought about destructive impacts to the supply and demand of international tourism, and that there are simple, yet constructive ways to reduce the impacts. First of all, different types of natural disasters have occurred around the world. Some previous ones included tsunamis and earthquakes. To begin with, a tsunami is a destructive wave that can travel at great heights and speed and is likely to damage areas near the coast. They tend to be unpredictable, and this can create difficult situations for those who are not prepared, which can result in deaths and cause destruction to surroundings. The tsunami that took place in the Sumatra Islands in Indonesia in 2004 is a clear example of how dangerous the impacts of a tsunami can be (Jain, Agarwal Hirani, 2005, p. 15).There have been cases where people were caught in a tsunami, and because of its unpredictable nature, unfortunate deaths have taken place. According to Jain et al. (2005, p. 15), The recent Sumatra earthquake the most devastating tsunami causing a death toll of more than 150,000. Therefore, tsunamis are dangerous and will continue to have devastating impacts unless necessar y precautions are taken to warn and advise people to move to higher grounds for safety (Jain et al., 2005, p. 17). In addition, earthquakes are also dangerous and are quite frequent. They are basically underground movements of the earth that travel up to the surface of the land or sea, which can trigger other natural disasters like a tidal wave. An earthquake can be destructive, and the impact it creates has a direct effect on the environment as well as on the country itself (Federal Emergency Management Agency [FEMA], 2011). An example, where an earthquake occurred would be in the Izmit city near Turkey, where Beirman stated Casualties were estimated to be 20 000 dead and 50 000 injured. According to a UN report, 350 000 housing units and business premises were damaged or destroyed. (2003, p. 162) The impacts of earthquakes can destroy infrastructure such as roads and buildings as well as public utilities such as electricity and water supply, the victims involved are prone to falling objects, death and injury including financial losses (FEMA, 2011). Second of all, the tourism supply is affected by natural disasters. With reference to Page and Connells study (as cited in Sessa, 1983, p. 59) found that the aim of the supply part is to make services available to tourists so that the tourism demands can be satisfied. Supply, provides many services such as; accommodation and transportation. With reference to the accommodation and transportation service, they will first show how they were affected during the tsunami in Indonesia, while attractions will be used as a discussion for the earthquake disaster in Turkey. The first supply is accommodation. Accommodation provides shelter for a tourist where he will be able to stay in while travelling to particular destinations near and abroad. The condition of the rooms provided should be comfortable and suitable in order to achieve a satisfied and happy guest, but if it was otherwise because of unsuitable room conditions then tourists will alternatively look for a better place to stay in while on holidays (Ahliya, 2010). For example, the accommodation in hotels and motels during the tsunami in Indonesia was badly affected, as close to 30 percent of room capacity was damaged (Athukoral Resosudarmo, n.d.) and even though some of the rooms were able to re-open for accommodation purposes, room occupancy rates were decreasing because of lack of infrastructure. This would lead to perishability, where expected numbers of rooms are to be sold within a time frame but the occupancy of rooms were less, resulting in financial loss (Walker, 2009, p. 11). Therefor e, the condition of a room and its quality after a natural disaster will have an effect on tourists choices and that will affect the financial wellbeing of the hotel and motel industry. Moreover, transportation is another supply of tourism. It helps people move from one place to another with a form of transport, such as an aircraft, car, boat or even a bicycle. Although transportation helps tourists travel to various destinations, they are dependent on the geographic location and the nature of infrastructure such as roads, airport runway and bridges. Constant improvements in these areas will create accessibility to places that are difficult to get to (Sorupia, 2005, p. 1768). Natural disasters do have an impact on transportation. The Indonesian tsunami in 2004 destroyed many roads and other transportation routes; which limited the public from travelling. The airport being one of the major ports that brings in international tourists was affected, and aircrafts were restricted to travel at certain times only to transport medical and food resources to affected areas (Samii Van, 2010). In fact, the demand side of tourism is also affected by the impacts of natural disasters. According to Page Connell (as citied in Pearce, 1995), the demand concept is focussing on the reasons as to what does or does not motivate a tourist to travel to a destination. Some of these factors may be due to personal income, destination image and health risks. First of all, personal income is one of the determining factors that influence a person in deciding whether to travel or not. With reference to price, if a person is able to afford travel expenses, then they are in a position to experience a destination and the attractions it offers, however, for those who cannot afford a holiday, it is a restricting factor that stops a person from travelling (Page Connell, p. 53). To illustrate this, Turkey experienced a growth in tourist numbers towards the twenty first century. Regardless of its low currency rate, it was considered to be a suitable and affordable destination to visit and this attracted many tourists from around the world (Beirman, 2003, p. 161). So, the income of a person will determine the willingness to travel to places if it can be afforded. Equally important, destination image is another factor that can also affect tourists demand. This is simply, the way in which a tourist views a destination, whether it is a suitable place to travel to. Some perceptions can be influenced by the media and tourists can instantly change travel plans, especially if it is a negative one and this can affect the status of a destination region (Lexow Edelheim, 2004, p. 52). For example, when the Izmit earthquake struck Turkey in 1999, there was coverage by the media which was showing how badly the country was affected by the earthquake. Yet, most of the information that the media circulated was later discovered to be biased and exaggerated, one such example would be the warning given out a day later by the British Travel agents where they were discouraging British tourists from visiting the largest city, Istanbul, assuming that the whole city was affected, however, only the east border was affected, while the inner part was not (Beirman, 200 3, p. 165).
